1. Call him back and ask to speak to a supervisor. Tell them you will not provide your ssn without them providing their EIN (the business equivalent of an SSN) and the name of their legal representative. If you need to fill out a tax form, do that separately and ask for the form separately rather than simply providing it on the release. Ask if you can provide another proof of ID.

2. Once you have the EIN, call the social security administration and check them out -- make sure they are legit and their information matches up with what you have from them.

3. Under no circumstances give out any credit card information or other financial info.

4. If at all possible, contact an attorney and ask them to verify the legitimacy of the release.

I would tend to think this is somewhat on the up-and-up, since you've had direct contact and they have not asked for payment or ccard info as yet. However, I would proceed with extreme caution -- as Cait said, she had to do the same for her contest... but it was clearly a legitimate business (a radio station) with an address and an EIN -- certainly she would have gone to the radio station itself and filled out the paperwork with a rep (Cait? Yes/no?). This is a little different. Is there any way for you to conduct the business in person?

Do you remember if you entered a contest for tickets?

Also - I don't know your age and don't wish to offend... but if you are not of majority age (18 or older) - you will need to have a parent or guardian execute the contract/release on your behalf. Also - the minute they start asking about credit cards or security deposits, hang up and run screaming in the opposite direction.
